Tax Exemption Application

Please do not place your order until you complete the following instructions as we cannot refund tax.

If you already have a tax exempt certificate on file with our Golden Valley Retail Store and Office:

The only way to have tax taken off your online purchase is for you to register a user account with our online store.

After you have registered or have an existing online account, please call our store at either (612) 799-8870 and let us know you are wanting to place a tax-exempt order on the Papa Hawks Sports & Collectible website. You must provide the staff member the email address you used to create your online registration account.

Our staff member will verify your ST-3 form and immediately go into your account and make the account tax exempt for your order.

Once the above step has been completed, you will need to refresh or reload your browser and then proceed to place your order.

As long as you log on before placing future orders, tax will always be removed.

If you do not have a Minnesota tax exempt certificate on file with our Minneapolis store:

After you have completed and signed the ST-3 form, fax it to our store at (612) ?.

After you have faxed the form, please call our store at either (612)-799-8870 or (612) 872-6666 and let them know you are wanting to place a tax exempt order on our website and that you just faxed over your ST-3 form. You must provide the staff member the email address you used to create your online registration account.
